The study examines medical information management in a combat zone—an environment characterized by stress, uncertainty, and high task load—where treatment success depends on information continuity across the evacuation chain.

The literature identifies a persistent gap in documenting and handing off information between echelons of care in the battlefield, leading to preventable errors. From a design and user-experience perspective, the study investigated how digital technology affects cognitive load and reporting quality. Using a Research-through-Design approach, a heuristic evaluation of the Blade Sheild 101 system was conducted, a user interface prototype was developed and tested among caregivers in the IDF’s Air Evacuation Unit, and domain validation was performed via interviews with physicians, paramedics, and medics who participated in the Swords of Iron War.

Findings indicate that documentation failure is systemic and fundamental, and that under extreme conditions real-time documentation collapses even with improved interfaces. The research contribution is a reconceptualization of documentation under stress and a reframing of medical documentation within the military care chain.
